PLEASE NOTE: Only use this form to register your child(ren) if you'd rather pay your fees via cash or check at a later time. Please Note: All Preschool - 6th Grades are required to have a uniform. You only need to purchase if you don't already have one.
Cubbies (Preschool) & Sparks (K-2nd) vests are meant to be worn over their clothes. They tend to run small. Consider ordering at least 1 size larger than your child wears currently. Uniforms can be worn more than one year so consider purchasing large enough to get more than 1 year of wearing.
NO Uniform is required for 7th-12th grades.

During the Awana year, Kindergarten - 6th grade clubbers earn points for participation and completing sections in their handbooks. Periodically, these clubbers will have the opportunity to "spend" their points in our Awana Store. If you would like to make a donation to aid in purchasing items to stock the store, please enter that amount in the following box. Thank you!
1) I understand that my child(ren) may participate in physical activities such as those held during Game Time. As with any physical activity, there is a risk of injury. I fully accept this risk and hold harmless from any legal liability, Federated Church of Bondurant, and any persons involved in the Awana Club ministry.
2) In the event of an emergency that requires medical treatment for the above named child(ren). I understand every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give my permission to the Awana volunteers to secure the services of a licensed physican to provide the care necessary for my child's well being. I assume responsibility for all costs connected to any accident or treatment of my child.
3) I grant permission for a photo of my child(ren) to appear in the club directory to be used by Awana leaders. I also give permission for the photo(s) of my child(ren) to appear among other general club photos that will be published within the Federated Church building (i.e. bulletin boards).
4) I grant permissioin for photo(s) of my child(ren) to be published on Federated Church's website or Facebook page. Names will not be published with the photos unless prior permission from the undersigned is obtained. I may opt out of this below.
5) I grant permission for my child to travel to/from Awana Club events with an adult leader. Any such event will be communicated beforehand.
